Eligibility criteria	All patients that meet Hospice criteria Any Individual with a life limiting condition or illness can be screened for Hospice appropriateness

Application process	Referrals are accepted from family, community and medical professionals. Hospice staff will follow up and secure the supportive documentation if patient meets Hospice criteria
Service Fees	Medicare, Medicaid, private insurance ***Services are also available for patients with no insurance coverage

Other comments	Our non profit foundation assists Hospice patients and their families with other financial issues based on individual needs. Team members include the services of a registered nurse, social worker, hospice aide, volunteer and chaplain are available.
