Community Care Council Priorities

   

Addressing Crisis Needs and Increasing Self-Sufficiency

1. Assistance with Immediate Needs including: Shelter, Food, Clothing, Financial assistance, Safety/security, Emergency medical/dental care, and Information and referral

2. Access to Services Enabling Self-Sufficiency including: Literacy classes, Credit/financial counseling, Employment coaching ,Public/reliable transportation, Affordable housing, Addictions treatment ,Therapeutic counseling, Mentoring and Advocacy

Building Strong Families and Nurturing Children

          1. Preschool: Increase the percentage of children that test ready for school

2. School Age Children: Reduce the drop out rate, teen pregnancy, alcohol and drug abuse and gang membership and participation

3. Adults/Families: Increase awareness and education level of parents to equip them to raise their children

Supporting Seniors

1. Education/Awareness: Retirement Planning and Financing, Healthcare and other services, Safety/Peace of Mind, and Protection from predatory lenders

2. Transportation/Access to Services: Bringing individuals to services and services to individuals

3. In Home Services (Aging in place): Community Centers, Home Repairs, Caregivers(Family)

4. Church and Community Outreach Services: Reaching all areas of the county   and serving those who “fall between the cracks”

Promoting Health and Healing

1. Addressing critical health needs in our community: Obesity, Smoking, Diabetes and Heart Disease

2. Awareness of Services to Potential Recipients: Reaching all parts of the county, Education & Prevention, Intervention for nutrition & physical activities

3. Access to Healthcare: Transportation to healthcare providers and taking services to outlying parts of the county

4. Case Management: Appropriate utilization of resources for targeted group, serving those that don’t qualify for services, and providing a clearinghouse function for those with multiple needs

         5. Access to prescription drugs

6. Promote the healthcare profession: Recruitment and retention, Support for first responders

7. Assistance for mental health consumers: Financial assistance, Programs and activities, Access to mental health services at time of crisis
